Important news regarding our practice closure on August 7, 2026.
Beginning August 10, 2026, the Stanford Medicine Children’s Health — Pediatrics office will be transitioning to a new location within San Ramon. While the physical location is changing, your child’s care will remain the same.
You will continue to see your child’s pediatrician, Ruchi Gupta, MD, Cynthia Quan, MD, or Nora Fahden, MD. While Dr. Gupta and Dr. Quan will continue to care for patients at another San Ramon location listed below, Dr. Fahden will continue to care for patients at our Pleasanton location listed below.

CHADIS Login
CHADIS is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office. After you complete the questionnaire, CHADIS results come directly to your doctor. The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.

If you are unable to complete CHADIS before your visit, please arrive 15 minutes before your appointment time in order to complete the health questionnaires in our office.

From I-680 North
Head northwest on I-680 North. Take the Crow Canyon Road exit. Turn right onto Crow Canyon Road. Turn right onto Alcosta Boulevard. Turn left onto Norris Canyon Road. Take the first right onto Hospital Place. Take the first left to stay on Hospital Place. Turn left, and 5601 Norris Canyon Road will be on your right.
From I-680 South
Head southeast on I-680 South. Take the Crow Canyon Road exit. Turn left onto Crow Canyon Road. Turn right onto Alcosta Boulevard. Turn left onto Norris Canyon Road. Take the first right onto Hospital Place. Take the first left to stay on Hospital Place. Turn left, and 5601 Norris Canyon Road will be on your right.
